General Summary: The Social Worker assists in providing direct care and management to all patients and their families including psychosocial program development; assessments; group therapy; individual therapy; family therapy; discharge planning utilizing appropriate resources; and active participation in the interdisciplinary treatment team.
Essential Functions:
- Performs bio-psychosocial assessments, gathering information from patients and significant others as appropriate.
- Direct patient care in clinical setting.
- Active participation of interdisciplinary treatment team.
- Assists with development of Master Treatment Planning and reviews of all patients.
- Assesses home environment and offers recommendations for treatment team to patient and family/significant others with assistance as needed in resolving issues that impact patient care.
- Utilizes all community resources recommended by treatment team to assist patient in discharge needs.
- Assists with the Partial Hospitalization Program problem identification and problem solving to provide continuous quality patient care.
- Continuously monitors patient care activities.
- Utilizes effective communication skills with staff, patients, and family members.
- Abides by Social Work Code of Ethics and policies and procedures of employer in providing quality patient care.
- Maintains confidentiality at all times.
Required Qualifications:
- A Master’s Degree in Social Work from an accredited School of Social Work.
- Must have clinical license in the State of Mississippi.
- A minimum of two years of psychiatric experience which includes group, individual, and family therapy, preferred.
- Must maintain current license.
Physical Requirements:
- Hospital, medical centers, health clinic office setting.
- Some travel is required.
