Community Health Needs Assessment
The purpose of the Community Health Needs Assessment (CHNA) report is to provide CRMC with a functional tool to guide the hospital as it works to improve the health of the community it serves. In addition, the report meets the guidelines of the Internal Revenue Service. The results of the CHNA will guide the development of health improvement initiatives and implementation strategies. The assessment was performed and the strategies were created by the Community Health Needs Assessment Steering Committee with assistance from HORNE, LLP. This describes the programs and activities that will address these health priorities over the next three years.
